Preparing for your consultation
For the most accurate assessment possible, please be sure to have a full summary of you or your child’s initial diagnosis on hand. In addition, please gather as much of the following information as you can:
- you or your child’s full name, address, and date of birth
- your insurance provider’s name (for example, Blue Cross Blue Shield of Massachusetts), the name of the specific plan (for example, HMO Blue), and your subscriber identification number
- the name, address, and phone number of the physician making the referral (if different from the primary care provider)
- a list of your child’s symptoms
- previous lab work reports
- any x-ray, MRI, or CT scan films or angiograms
- Please call the center at 617-919-1379. We may request additional images once we’ve been in contact with you.
Remote image upload
It is helpful for us to have the following test results when first reviewing the patients case, along with:
- any x-ray, MRI, or CT scan films or angiograms
- relevant lab work reports
Images may be uploaded to our secure server. For temporary credentials and instructions, please e-mail CVDSurgery@childrens.harvard.edu or call 617-919-1379. We may request additional images once we’ve been in contact with you.
Videoconferencing
For patients who live far from Boston, we can often conduct follow-up visits through telemedicine. Contact CVDSurgery@childrens.harvard.edu or 617-919-1379 to arrange.
