- Travel itinerary (dates and locations of where you will go and where you will stay while traveling)
- A copy of everyone's immunization record who will have an appointment with the clinic
- Lists of medications you or your child is taking
- Anyone under the age of 18 who is scheduled to receive an immunization with the clinic must be accompanied by a parent or legal guardian.
Appointments are recommended at least four weeks prior to traveling in order to complete all the necessary travel immunizations and counseling.
Before visiting the Travel and Geographic Medicine Clinic or departing for travel, we highly recommend you visit the Center for Disease Control and Prevention's website, as it is full of pertinent information regarding regional diseases and how to minimize potential risk.
